Organising care after a hospital stay
A practical checklist for the period between discharge, arriving home and the first planned visits.
2 minute readClarify the discharge plan
Ask for written discharge information, planned follow-up and any existing instructions. Record which service can be contacted for medical questions; VITANIS does not replace medical follow-up.
Review daily life at home realistically
Consider where help will be needed in the first days: personal care, mobility, meals, daily routines or accompaniment. A clear overview avoids assuming that visits take place automatically every day.
Identify professional nursing needs
Wound care, medication management and other nursing tasks are assessed separately. Existing instructions and professional requirements must be clarified before a service can be confirmed.
Record the period, hours and costs
Recovery is time-limited and distributes agreed total hours across the booked period. Services, nursing allowance, frequency, materials and possible extra hours are agreed in writing before any binding engagement.
Respond appropriately to changes
If support needs change, the plan is reviewed together. Acute symptoms or an emergency require the appropriate medical service or emergency service; a website cannot provide an individual medical assessment.
